Q: Night guard or occlusal splint: what's the difference?

A night guard is a slim protective shield for everyday grinding, while a hard occlusal splint is a rigid, precisely adjusted appliance for more serious problems with your bite. The splint, from £620, is refined over several visits and is the right choice for severe wear, forceful clenching or a painful jaw joint, where calming the muscles counts as much as protecting the teeth. For mild to moderate grinding a night guard, at £210, is generally sufficient. After examining your teeth and jaw, your dentist will advise which one you need.

Q: Are custom mouthguards better than boil-and-bite ones?

Yes. A custom guard fits more precisely, gives better protection and, crucially, actually stays in the mouth during play. Built from a scan of your own teeth, it's slimmer than a boil-and-bite version, stays in place without you having to clench, and leaves you free to breathe and call to teammates. It also distributes the force of an impact more evenly over the teeth. Guards bought off the shelf tend to be bulky and loose, which is why players so often take them out; ours cost £190.

Q: How long does a mouthguard last?

A sports mouthguard typically sees out one or two seasons, though it needs replacing earlier if your teeth shift, after any dental treatment, or as a child's mouth develops. At the start of every season, inspect it for splits, thinning and a fit that's gone slack. A night guard wears down more slowly and will often serve for several years, although heavy grinders go through them more quickly. We look at night guards during your examination, and once a crack appears or a patch has worn right through, it's time for a replacement.

Q: Can a dentist help with snoring?

For simple snoring, frequently yes. A custom anti-snoring device, from £620, keeps your lower jaw positioned a few millimetres forward during sleep, so the airway is less inclined to collapse and vibrate. Made from a scan, it's adjusted so that it doesn't put strain on your jaw joints. Heavy snoring accompanied by pauses in breathing, choking or waking up unrefreshed may point to obstructive sleep apnoea, and your GP should organise tests for that first. If the sleep clinic decides an oral appliance would be useful, we can make one for you.

Which Appliance Do You Need?

Side by side they look much the same, yet each has its own purpose. We'll suggest the right one once we've examined your teeth, bite and jaw.

Sports Mouthguard

Moulded to your own teeth for contact sport, whether that's rugby, hockey or football, or boxing, judo and the other martial arts. The snug fit lets you breathe and talk normally, so it stays in during training rather than in your kit bag (£190).

Night Guard

A slim, clear appliance you wear to bed. It absorbs the load from clenching and grinding, sparing your natural teeth and any crowns, veneers or bonding you've had done (£210).

Hard Occlusal Splint

A firm acrylic splint, refined across several appointments until your bite meets it evenly all round. The choice for severe grinding, obvious wear or jaw pain (from £620).

Anti-Snoring Appliance

A pair of connected trays that bring the lower jaw gently forward overnight, opening the airway for straightforward snoring (from £620).

Retainers

Do You Grind Your Teeth?

Because most of it goes on while you're asleep, few people are aware of it. The signs below are a better guide.

Aching or weary jaw muscles on waking, or a dull headache at the temples in the morning

Crowns, fillings or bonding that repeatedly crack or fall out

Little notches at the gum line, or teeth that have started to feel sensitive

How It's Made

Examining Your Teeth and Jaw

We look for wear facets, cracks and tension in the jaw muscles and joints, and rule out anything that should be dealt with first, like a fractured filling or a tooth that meets too early.

A Few Minutes with the Scanner

Both arches are captured by the iTero Lumina in a matter of minutes. The digital file goes to a dental laboratory, which makes the appliance.

Fitting It

Once it's back from the lab we seat it, make sure it neither rocks nor pinches, and trim it so your teeth meet it evenly.

Checking Back In

After a few weeks of use we see you again to confirm it's comfortable and doing what it should.

Shop-Bought Guard or Custom-Made?

A boil-and-bite guard from the chemist will do as a short-term measure. The problem is bulk and a loose fit, so most end up abandoned in a drawer after a few weeks, and a guard protects nothing from a drawer. A custom-made one is thinner, holds onto the teeth and is adjusted to suit your bite.

It's worth being honest about what a night guard can do. It protects the teeth from the grinding, but it doesn't cure the grinding itself. Where stress, broken sleep or a problem in the jaw joint is driving it, we'll tell you and point you towards the right help. Snoring appliances are for ordinary snoring only. If your breathing pauses in your sleep, you wake gasping, or you doze off through the day, sleep apnoea may be the cause. Speak to your GP first; a dental appliance may then be part of the treatment they set up.

Looking After It

Each morning, rinse it in cold water and clean it with a soft toothbrush. Hot water will distort it

Allow it to dry in the air, then store it in its box, out of direct sun and beyond the reach of pets

Take it along to your examinations so we can check how it fits and where it's wearing

A hole worn through it means it has done its job, and that it's time to replace it

A child's sports guard has to be remade as the jaw grows and adult teeth arrive

Do I need a night guard?–

Quite possibly, if you wake up with a sore jaw or a headache, your teeth look worn down or chipped along the edges, or your fillings and crowns keep fracturing. Sometimes a partner hears the grinding at night, but plenty of people only learn about it when we notice the wear during an examination. A guard is particularly worthwhile if you've invested in veneers, bonding or implants, because it shields work that cost a great deal more than the guard does.
